1) Take employee to medical provider for treatment
2) Complete and file the:
Supervisors/Employees
incident report form
Witness report form
(if applicable)
3) Upon Receiving the medical bill, fax a copy to Caryn Hall (916) 781-5711
of Intercare Insurance Solutions for bill review. She will send you the
recommended allowance to be paid. Send the adjusted payment to medical
provider with a copy of the bill review.
